1. Introduction

By nominating a community pharmacy for Electronic Prescription Service (EPS) prescriptions, you agree to adhere to the following terms and conditions. These terms are designed to ensure the safe and efficient management of your prescriptions through the EPS system.

2. Nomination Process

• 2.1. You have the right to nominate any community pharmacy in England to receive your EPS prescriptions.
• 2.2. Nominations can be made through your GP practice or online via the NHS website.

3. Consent

• 3.1. By nominating a pharmacy, you give consent for the pharmacy to access and manage your prescription information.
• 3.2. You understand that your nominated pharmacy will have access to your electronic prescription record.

4. Prescription Dispensing

• 4.1. The nominated pharmacy will dispense your prescription in accordance with the prescription issued by your GP.
• 4.2. The pharmacy will ensure that the medication dispensed is appropriate for your condition and in compliance with relevant regulations and guidelines.

5. Privacy and Data Protection

• 5.1. Your personal and prescription data will be handled in accordance with applicable data protection laws.
• 5.2. The pharmacy will use your data solely for the purpose of managing your prescriptions and providing you with pharmaceutical services.

6. Prescription Collection

• 6.1. You are responsible for collecting your prescribed medication from the nominated pharmacy within a reasonable timeframe.
• 6.2. Failure to collect prescriptions may result in medication being returned to stock, and you may need to request a new prescription from your GP.

7. Prescription Expiry

• 7.1. Some prescriptions may have an expiry date. It is your responsibility to ensure that you use the prescription before it expires.

8. Change of Nomination:

• 8.1. You can change your nominated pharmacy at any time by contacting your GP practice or using the NHS website.
• 8.2. Changes may take up to 48 hours to process.

9. Complaints

• 9.1. If you have any concerns or complaints about the service provided by the nominated pharmacy, please contact the pharmacy manager or the NHS.

10. Termination

• 10.1. You have the right to terminate your nomination at any time.
• 10.2. The pharmacy also has the right to cease providing services if it deems it necessary, following proper notification procedures.

11. Amendments to Terms and Conditions

• 11.1. These terms and conditions may be updated from time to time. You will be notified of any changes in advance.

12. Governing Law

• 12.1. These terms and conditions are governed by the laws of England and Wales.

By nominating Tyrrell and Jones as your community pharmacy for EPS prescriptions, you acknowledge that you have read, understood, and agreed to these terms and conditions. Your cooperation ensures the safe and efficient management of your prescriptions through the Electronic Prescription Service.

1. Nominations

You agree to nominate Tyrell and Jones as your preferred pharmacy.

2. Consent

You are consenting to receiving medication in medication compliance aids (Dossette Boxes).

3. Onboarding

A member of the pharmacy team will be in touch within 5 days of subscribing for the service.

4. Delivery

There are no additional charges for medication delivery.

5. Fees and Payment

A payment of £25 will be taken via monthly direct debit. This is a recurring payment. No payment will be taken until onboarding is complete.

6. Termination

You can terminate this subscription at any time.

7. Complaints and Disputes
